Per-workspace SLA enforcement + reviewer efficiency dashboard

The slo-watchdog cron now reads per-workspace SLA configs instead of using hardcoded 30m defaults. When autoEscalate=true, submissions approaching the warn threshold trigger a direct notification to the designated reviewer or broadcast to all ADMINs. The new /sla page shows compliance rate, p50/p95 latency, breach count, and a per-reviewer efficiency breakdown.
